How to Apply for NDIS
Are you facing temporary or permanent mental or physical challenges? You could qualify as an NDIS participant! Take these easy steps to seize your opportunity for additional assistance through the NDIS.
1. Complete the 'Application Request Form'
Complete the form provided to start your NDIS application.
2. Visit Your GP or Specialist:
Schedule a visit to your doctor or another professional for Section 2 of the Application Request Form.
3. Gather Important Medical Reports:
Collect recent and relevant medical reports that can support your application.
4. Collect 100 Points of ID:

5. Submit your application:
Choose one of the following options to submit your completed application: Visit your nearest NDIS local office.
